Hylenex® What is it and why do we need it on hand at all times?

Emergent:


In the event of injection of a hyaluronic acid product, such as Restylane®, Juvederm®, Versa®, RHA® or Belotero Balance® into a vessel (vascular occlusion) or too much product outside or next to a vessel (vascular compression).

The amount of Hylenex® product used in this situation is typically exponential in comparison to the amount of product that caused the issue or even in comparison to elective dissolving. If product is left causing a vascular event, necrosis & tissue loss can happen. Depending on areas injected at the time of the vascular occlusion, blindness or stroke may occur. Anatomy, product rheology and injection techniques are paramount to safe and effective outcomes. There are higher risk areas in which we do not inject at our clinic, as we believe safety outweighs the risk in those areas.

Elective dissolving: Sometimes filler settles in a space it wasn’t intended. Sometimes patients become overfilled over years and sometimes it’s just best to start fresh. I see a lot of patients who come to me needing a full dissolve, most commonly of the lips. The amount of dissolving sessions depends on the product used, how much was used and how long ago the product was placed.

A treatment plan is determined between physical exam, history taking (best to get your notes from previous clinics) and also how the product responds session by session. You can dissolve once per week until we remove all the unwanted product. If refilling is desired, we must wait a minimum of 2 weeks to refill after dissolving is complete.
